The process is simple, but requires 5 steps. 

1. Text me, Dr. Eskandari, at (704) 200-8092 to request Sick Care services.

2. You will receive a response with whether or not I am available. I currently also work at a local emergency department and will have periods of time I will not be available. If I am available, I will respond with my next available time.

3. Review all these “important information” dropdown sections on this website, and text me when you are finished and if you would like to move forward with my services. I will also give you a brief call to ensure that Sick Care is likely to be appropriate for your needs, which is free of charge.

4. I will email you a link to the Electronic Health Record patient portal registration where you can sign up and fill out the pre-visit form including informed consent, past medical history, medications, allergies, and preferred pharmacy information.

5. I will come to your home and evaluate and treat your condition as appropriate, after which payment will be collected via credit/debit card or health savings account (HSA) card.

Your records will be available on the Electronic Health Record via the Patient Portal after your visit.

I can handle many minor illnesses and injuries which do not require advanced diagnostic testing including:

• Minor lacerations/cuts which may requIre stitches or other closure

• Migraines which may require IV migraine cocktail

• Gastroenteritis (often also called stomach flu, stomach virus, or food poisoning)

• Urinary Tract Infections

• Ear infections

• Minor abscesses which may require drainage

• Minor burns

• Nausea and vomiting

• Sore throat (including possible strep)

• Sinus infections

• Non-severe asthma exacerbations

• Children’s fever

• Rash (excluding genital rash)

• Back spasms or pinched nerve

• Mild dehydration

The model of providing healthcare in patients homes has inherent limitations. It is meant for minor illness or injuries, and not designed for serious illness or injuries. I can perform minor procedures and have some limited access to diagnostic testing. My testing is currently limited to the following: rapid strep, rapid COVID, rapid flu, blood glucose finger stick, urinalysis (urine dip), and urine pregnancy test.

Due to the nature of home visits, I do not perform the following:

• Genital exams

• Delivering babies

• Routine prenatal care

• Screening for sexually transmitted infections.

• Mental health care including psychiatric medication management or crisis

• Prescribe controlled substances or narcotics

• Sports physicals

• Emotional support animal letters

• DOT physicals

I also do not currently have capability for advanced testing, including but not limited to:

• Xrays

• Blood tests

• Administration of narcotics

• Cross sectional imaging including CAT scans

For a limited time only, visits are a promotional flat cost of $295 (typically $495) which includes everything without any surprise bills or charges. This includes my travel to your home, evaluation, any necessary testing, procedures, and treatments administered during your visit. You will also be provided with prescriptions if appropriate for your problem.

Costs outside of Sick Care, such as filling prescriptions, Emergency Department costs, EMS costs, or cost of followup appointments are not included.

If your problem requires that you be referred to the ER for more advanced testing or care than can be provided in the home, my visit will be free because that is how it should be.

You can pay with a Health Savings Account credit or debit card (Visa, Mastercard, Discover, or American Express), and payments may count towards an annual deductible for out of network care depending on your insurance. However, I am not currently accepting health insurance payments. I also accept payment by credit or debit card, not cash.

Available hours are currently variable because in addition to launching Sick Care, I am also working full time in a local Emergency Department. Care outside of business hours is often available, just send me a text to find out.

No, I do not carry or otherwise provide them in this context.

I will do my best to address your concerns within the scope of an at-home visit. Although unlikely, it is possible that after I come out to assess and treat you, it may become clear that you need more than can be addressed in an at-home visit. If this happens, I will assist you in finding ER options, as well as call ahead to speak to the provider at the facility etc. as appropriate. As noted above, this will be free of charge.

If you do not feel that I have adequately addressed your problem or concern, I will not charge you.

There are inherent risks to providing healthcare anywhere, including some risks unique to the home environment.

I take steps to mitigate these risks. Although I stress this is not an emergency service, I do carry some emergency equipment including supplemental oxygen, and an AED in the unlikely event of an emergency.

Additionally, there are no standardized sanitation procedures in the home environment. In order to mitigate infection risk, I use single-use, sterile equipment for anything entering the skin, just as you would expect to see in a healthcare facility.

Despite the risks, there are also some benefits to receiving in-home care. The home is less likely than a healthcare facility to expose you to drug resistant bacteria including so-called "superbugs", and less likely to expose you to an unrelated infection such as COVID, influenza, or C. diff.
